SERVICES

HFIN has agreements with local governments and other partners to provide services free of charge to eligible families. Primary funding to provide the following services is through the Arizona First Things First program which seeks to improve early education and health ser­vices for children 5 years old and younger:

Outreach: HFIN reaches out to families that are likely to qualify for public health insurance through a variety of venues, including: distributing outreach materials and making presentations at community events, such as health fairs; renewing, maintaining and establishing new contacts with community partners, such as other service providers and schools; maintaining contact with local government officials; maintaining contact with individual neighborhood residents; and maintaining scheduled, reliable hours of service at designated local sites.

Outreach Materials: HFIN designs and updates outreach materials for distribution at key community sites. These materials are designed to be easily understood, with basic information conveyed in both English and Spanish.

Identifying Eligible Families: The targeted population is families with children aged birth through five who reside in the Southwest Maricopa region and are likely to qualify for public health insurance. Our focus includes uninsured and underserved families and/or geographic locations that are considered underserved, especially families with incomes at or below 200% of the Federal Poverty Level (FPL).

Application Assistance: We provide assistance to families that will likely be eligible for assistance, including information, encouragement and education on available programs, including public health insurance through the Health-E-Arizona application.

Renewals: We provide individualized follow-up services with families and DES liaisons to ensure that families to not lose their health insurance benefits once they have been approved.

Referrals: Working with a broad network of community partners, HFIN staff makes referrals to other community agencies that provide services such as food and nutrition, housing, job placement, education and training and more.

HFIN conducts all of its business in an atmosphere of warmth and compassion for the individuals and families it serves, as part of an overall culture of excellence. Primary goals are to ensure that every person served feels respected and cared for as an individual and to secure optimum benefits for each person served.
